Altered Mental Status During Critical Illness: Delirium and Coma

چکیده
altered mental status secondary to their acute disease processes or as a consequence of the therapies used to treat disease. These mental status changes range from coma to hyperactive delirium. A comatose patient is unresponsive to physical or verbal stimuli, whereas delirium is an acute and fluctuating disorder of consciousness characterized by inattention, disorganized thinking, and perceptual disturbances (Figure 1). Alterations in mental status have traditionally been considered expected consequences of critical illness, and clinicians are increasingly aware that these mental status changes are manifestations of acute brain organ dysfunction that are associated with worse clinical outcomes. Early studies evaluating coma and delirium were hampered by the many different terms (eg, confusional state, ICU psychosis, acute brain dysfunction, and encephalopathy) used to describe altered mental status during critical illness. Additionally, the lack of validated bedside tools (besides the comprehensive Chapter 1

Ornithine Transcarbamylase Deficiency: If at First You Do Not Diagnose, Try and Try Again
Ornithine transcarbamylase (OTC) deficiency is well known for its diagnosis in the neonatal period. Presentation often occurs after protein feeding and manifests as poor oral intake, vomiting, lethargy progressing to seizure, respiratory difficulty, and eventually coma. Presentation at adulthood is rare (and likely underdiagnosed); however, OTC deficiency can be life-threatening and requires pr...

Altered mental status is a common chief compliant among older patients in the emergency department (ED). Acute changes in mental status are more concerning and are usually secondary to delirium, stupor, and coma. Although stupor and coma are easily identifiable, the clinical presentation of delirium can be subtle and is often missed without actively screening for it. For patients with acute cha...

This article reviews the significance of altered mental status in older emergency department patients. Specific diagnoses are discussed, including delirium, stupor and coma, and dementia, with a focus on delirium. Finally, an approach to all older patients is suggested that should result in increased clinician comfort with older patients, improved ability to communicate with other physicians, i...
متن کاملManagement of pulmonary vasodilator therapy in patients with pulmonary arterial hypertension during critical illness
Pulmonary arterial hypertension (PAH) is commonly treated with pulmonary arteriolar vasodilator therapy. When a patient on PAH medication is admitted to intensive care, determining how to manage their medication during the critical illness is often complicated. There may be considerations related to the inability to take medication by mouth, related to acute renal failure or acute liver injury,...
متن کاملAssociation between endothelial dysfunction and acute brain dysfunction during critical illness.
BACKGROUND Acute brain dysfunction (delirium and coma) during critical illness is prevalent and costly, but the pathophysiology remains unclear. The relationship of acute brain dysfunction with endothelial function, which is impaired in critical illness and may contribute to alterations in cerebral blood flow and blood-brain barrier permeability, has not been studied. This study sought to deter...
